iMPORTANT

  • Your full name MUST match the name found on your passport including your middle name(s) and title such as Ms, Miss, Mrs, Dr or Mr.
  • If you've traveled with Cunard before, we MUST have have your Cunard World Number (found on your last booking confirmation).
  • Your form will NOT be submitted if you fail to enter details into the RED *ASTERIX* BOXES. Please make sure each of these boxes has the correct annotation, even if it is N/A or NONE. There are checked boxes also put in place at the very bottom of the booking form to make sure the form is correctly submitted.
  • You MUST select SUBMIT for the form to send correctly.
  • Incorrectly filled boxes will be highlighted in red. 
  • Forms with missing information will NOT be submitted to Cunard.
  • PLEASE complete one form for each stateroom.

     If You've Travelled With Cunard Before, Please Include Your Cunard World Number. This speeds up the booking process considerably. This can be found in your previous booking confirmation emails (it is 8 characters long and starts with 2 letters, followed by 5 numbers and a final letter). Please Note: Maggie does not know this as she never received an email copy of your booking. This will also increase your chances of a potential free upgrade.
